Welcome to ADAPT Lab Research Team!


We are so glad to have you join in our important research.  Please complete the following list of items and upload all your documents upon completion to the Smartsheet link at the bottom of the list. UGRAS must complete each step.

  1. Sign and return the Confidentiality Form  (PDF Jen will send)
  2. Get a TB test and upload the copy of the negative results (Test can be done at Watkins) 
  3. Complete  CITI certification (basically a short course on research dealing with human subjects research issues, like confidentiality).  The CITI certificate training course is a series of modules about human subject protections, confidentiality, data management, etc. with short quizzes at the end.  You can begin at the CITI certificate training course site. Click on the “KU Human Research Protection CITI tutorial” and log in. You may need to create a new account to get started.  You only need to complete the Social & Behavioral Research module.  Once you have completed the training and quizzes, you will receive a certificate that is good for 3 years.  Since the training will take a little bit of time, you can save and come back to it section-by-section.  As soon as you have finished the training, upload a PDF copy of your certificate.   
  4. Complete the State of Missouri criminal background check – the process should take less than 10 minutes.  The cost of the background check is approximately $14, for which you will be reimbursed. You may complete the background check at the State of Missouri portal.  Specifically, select the Name Search Portal (and not the Fingerprints-based Search) to start the process. Once you have completed the submission process, you will receive a confirmation number and a PDF certifying your status—attach and send the PDF certificate directly to me. 
  5. Upload photograph of yourself for the NIH Requirements 
  6. Fill out this form and submit all application materials.
